Here, at Royal Crest Centre for Advanced Dentistry®, our doctors realize there is a personal connection of trust between themselves and their patients. It is very important that you feel comfortable in our office and with our staff. We want you to feel that you can express your dental needs with us in order for us to have a clear understanding of what it is you want to achieve, but also so we can make sure we are providing you with the most comfortable visit possible, so your dental experience is positive. Because of the trust level we have with our patients, our Dentists only use the referral system of specialists for what they are supposed to be used for. We only send patients to a specialist in cases where their equipment or expertise will help when there is a unique case or a highly complicated procedure that they have been trained to handle. We are one of the only dental offices in the upstate that take pride in fact that we minimize the amount of referrals to specialists. Most dental offices in the upstate choose not to offer as many services as we do therefore they have a much higher referral rate to specialists. We value our doctor/patient relationship and feel that referrals interfere with that. It is Dr. Marcello’s feeling that general dentists are taught to be able to perform all aspects of dentistry and should only be using the referral system for unique cases. Too many dentists do not provide the community with the services they have been trained in school to perform.
Many people do not understand that general dentists are licensed to perform many different procedures in their office, if they chose, whereas specialists are limited to only the specialty that they have spent extra years of education to practice. For instance, if you need an extraction and a filling done, you cannot go to an oral surgeon to have both procedures done—only the extraction. Eventhough the oral maxillofacial surgeon has learned through his educational process how to perform the filling service, since he has specialized in oral surgery, he can only perform the extraction in this case, not the filling. Our office of general dentists can take care of the extraction and the filling in the same office for you because our dentists are not limited to a specialty. This is helpful to our practice in many ways. Our practice offers many services in one location for our patients; and, the following chart describes what our office offers and what specialists offer so you can better understand what services are possible in a general dentist office. Our general dentists are very confident in their capabilities and limitations and know when they need to refer out to a specialist who has the training or equipment necessary for certain cases. But again, our highly trained dentists reserve only very difficult cases to be referred out so we gain trust with our patients, and so we can shorten your treatment by doing as many services at once as your time is valuable. It is a common misunderstanding that only an oral surgeon takes out wisdom teeth. As you can see on the chart, our office can not only sedate you for your wisdom teeth extractions, but we have an x-ray system that can show us important structures, such as nerve placement, if we need to look at your mouth in 3D before the surgery. Many of the specialists send us patients who may need fast service if the specialist is fully booked, or maybe if we can combine our services to help the patient, such as sedating a child for a pediatric dentist to treat all dental issues the child has in one visit. We strive to be a resource for the specialists, just as they in turn are a great resource for us when we need their services.

Oral and maxillofacial surgeons are dentists specializing in surgery of the mouth, face and jaws. After four years of dental school, surgeons receive four to seven years of hospital-based surgical and medical training, preparing them to do a wide range of procedures including all types of surgery of both the bones and soft tissues of the face, mouth and neck. They are a very good referral source for us, and if we determine that you have a tooth that is a unique or a complicated case, you can trust that when we send you to an oral surgeon, it is a condition that needs their expertise.
We appreciate that oral surgeons send us patients when they are booked out and a patient needs an extraction or wisdom teeth removed right away, as they know we are able to get patients in quickly and our capabilities allow us to help. Unlike other general dentists, we work in close conjunction with our oral surgeon dentists, and we receive many referrals from our oral surgeon colleagues for patients who need many types of dental treatment, which is highly unusual in the general dental community.
Periodontists are dentists who specialize in the diagnosis and treatment of periodontal (gum) disease. They have had extensive training with two additional years of study after dental school. As specialists they devote their time, energy and skill to helping patients care for their gums. They are a very good referral source for us if we determine that you have advanced periodontal disease or gum conditions needing a specialist. The periodontists we chose to send you to care greatly for our patients and we commend them on the services that they provide in conjunction with ours to help our patients retain the goal of healthy gums and rid them of gum disease.
Your dentist has determined that your gums require special attention or surgery that is more complicated. The periodontist and dentist work together as a team to provide you with the highest level of care. They will combine their experience to recommend the best treatment available to you while keeping each other informed on your progress. By referring you to the specialist, your dentist is showing a strong commitment to your dental health.
The Endodontist is a specialist that is used to diagnose teeth that have damaged or dead nerves in them. Once diagnosed with this particular condition, an endodontist performs what is known as a “root canal” on a tooth. The root canal is the only means by which the tooth can be preserved once the nerve goes bad. We typically utilize an endodontist in situations where it is extremely difficult to diagnose which tooth is creating a problem or it has been determined that the roots of the teeth in question could be extremely difficult to treat with a root canal.
The prosthodontist examines and diagnoses disabilities caused by loss of teeth and supporting structures. They formulate and execute treatment plans for the construction of corrective prostheses to restore proper function and esthetics of the mouth, face, and jaw. A prosthodontist should not be used for regular dentures or partials. We will send patients to a prosthodontist on rare occasion for rare disability treatment.
A pediatric dentist has at least two additional years of training beyond dental school. The additional training focuses on management and treatment of a child’s developing teeth, child behavior, and the special issues involved of children’s dentistry. Although a general dentist or a pediatric dentist is capable of addressing your child’s oral health care needs, a pediatric dentist is trained to care for special cases of children and to put them at ease. Pediatric dentists are trained to mainly treat and preserve baby teeth in children. They also have training to be able to perform simple restorations on adult teeth also.
Many pediatric dentists send their patients to Dr. Marcello on a daily basis for treatment referrals because of situations that arise in children where the treatment needed for the patient is beyond the capability of a pediatric dentist. Previously, children under the age of four were referred to a pediatric dentist for care and some had to go to a hospital for serious dental treatment, as our office does not restrain children and could not perform work that restraining devices used in a pediatric office are necessary for. But, with the addition of Dr. Pappas who provides hospital level sedation services in our office, the use of restraint is no longer necessary, the need for taking your child to a pediatric dentist who goes to the hospital to perform dental work, is now not necessary, we have taken away the burden of the large hospital cost for you, and all services needed can be done in the comfort of our office with our caring dentists.
An orthodontist prevents and treats mouth, teeth, and jaw problems. Using braces, retainers, and other devices, an orthodontist helps straighten a person's teeth and correct the way the jaws line up.
Orthodontists treat kids and adults alike for many problems, including having crowded or overlapping teeth or having problems with jaw growth and tooth development. These tooth and jaw problems may be caused by tooth decay, losing baby teeth too soon, accidents, or habits like thumb sucking. These problems also can be genetic or inherited.
Orthodontics is the one service that Dr. Marcello does not offer in our office. There is a lot that is involved with braces and he wants your orthodontic outcomes to be perfect, so we do refer our patients to our orthodontists for help with problem teeth. Our orthodontists are also hand chosen by our dentists and we have a great working relationship with them. If you need a tooth removed as a part of your orthodontic treatment, you will have that done in our office and the orthodontist will then put on your braces. Many orthodontists refer patients to us for extractions, as they know we can get patients in quickly so they can continue with the orthodontic treatment with you promptly.
